It covers 24/7 call coverage, same day availability for office visits or phone consultation, phone consultation addressing medical needs that may arise during travel and availability to consult with family members as necessary or as requested.
The fee is needed to re-establish independence from intrusive third parties, such as health care systems, insurance companies, and the government. It allows the practice to operate with full focus on the patient's needs and concerns. It removes the intrusion of the administration requirements and time demands of these third parties. It allows the doctor to care for a smaller cohort of patients, freeing care from a rushed, hectic, time pressured environment driven by the insurance company or the health system. In short, it allows for personal, relaxed, and focused comprehensive care directed by your needs and concerns.
Yes. You will maintain insurance coverage just as you do now and it will still pay for all necessary medical services, such as labs, radiology, procedures, and hospitalization, just as it does now. We will submit a bill to your insurance for all in office care in order to be a recognized provider of care, so that all other services ordered by the doctor will proceed as seamlessly as possible.
The annual fee is $1500 per individual and $1200 per each additional family member. It is preferable to pay annually as there is a cost savings. However, if you choose to pay monthly the fee will be $140 per month.
We hope to have an open, honest, and respectful relationship with you. If there are problems we cannot resolve together, then we will gladly accept a written notice of your desire to seek care elsewhere. We ask for a 30 day notice and we will reimburse you the prorated remainder of the CM fee.
I am an ABIM board certified general internist with over 25 years of experience. The scope of my practice is comprehensive adult medicine and I can manage most primary care medical problems in the office with only occasional need for specialty consultation. I have many years of experience in managing diseases of the heart & vascular system, lungs & respiratory system, gastrointestinal tract, bone & joints including osteoporosis, nervous system, kidney & urinary system, thyroid & other hormonal systems, and skin. I also can manage most common mental health problems such as anxiety, depression, and OCD.
Absolutely, that is why I am on call. If you have a problem that you feel cannot wait until the morning, or are uncertain whether you should wait until then, please call my cell phone number so that we may decide what is best. If it is an emergency, then call 911.
You will receive care from the hospitalist group associated with your hospital, just as you would now. I will be available during your hospitalization to provide needed information, answer questions that may arise, and also coordinate all post hospital care.
You can call me in the office, or on my cell phone after hours, and I can assist you to fullest extent possible.
My practice will be covered by a physician colleague of mine. I will remain available if they feel the need to contact me with regard to your care.
